Please use this form to submit your request for a Radiology & Imaging appointment. The fields colored in red are mandatory and must be completed in order for your request to be submitted. Our Central Scheduling Office will respond to all internet inquiries Monday through Friday between the hours of 7:30 a.m. and 7:00 p.m.

You will receive an e-mail confirming receipt of your request, and we will contact you by telephone to schedule your exam(s). The information you provide will be kept strictly confidential and will not be shared with any third parties.

Please note: Imaging procedures require a physician's prescription. Please bring your prescription with you on the day of your appointment. For questions related to your prescription(s) or the reason why your doctor is requesting the exam(s) please contact your doctor directly.

Some procedures require pre-authorization from your health insurance carrier, which your doctor must provide. Please check with your insurance provider.
