The Social Work Department serves acutely and chronically ill patients and their families of all ages Through the continuum of birth to death who have psychosocial problems related to their medical situation, hospitalization and & future care needs. In addition, we count as our customers, our physicians, nursing staff and all ancillary and support services as well as the community agencies and facilities we work with in order to accomplish our goals.
The functions and services provided by the Social Work Department include the following:
- Assessment of the patients' family's psychosocial and/or financial needs
- Individual and family counseling/education intervention
- Bereavement counseling
- Counseling around adjustment reaction to dialysis
- Nursing home/Physical Rehabilitation referrals
- Boarding home/Assisted living referrals
- Referrals for financial assistance
- Referrals for follow-up counseling/support groups
- Referrals for community services
- Supportive counseling for the terminally ill patients/families
- Adoption counseling and placement
- Child abuse/neglect referrals
- Child/elder protective service referrals
- Counseling education and collaboration with DYES for the new mother with substance abuse
- Counseling and support services for the AIDS patients and their families
- Assessment and development of complex post-discharge plans
- Patient/Family advocacy
- Support groups as needs are identified
The Social Work Department hours are Monday through Friday 8:00 a.m. - 4:00 p.m. with some support groups running in evening hours. In the case of a medical social work crisis during evenings, nights or weekends, the Social Work Manager may be contacted through the Medical Center operator. The Emergency Department Social Worker may be called to assist if their caseload allows.
