Medical Records

Medical Records is located at Muhlenberg. If you need a copy of your medical records for a physician, there is no charge to make a copy of your file. If you need a copy of your medical records for your own files, there is a $1.00 per page charge up to 100 pages and then a .25 cent per page charge up to $200.
